Shift differential for PM ($1.25/hr.), NOC ($1.50/hr.), and Weekend ($3.00/hr.)

Who you are:
- Current licensure by the California Board of Registered Nursing
- Current CPR certification
- Two years in long-term care with prior charge nurse experience preferred
- Experience with wound care and admissions strongly preferred
- Knowledge of CMS regulations and long-term care standards
- Strong assessment, organizational, and communications skills
- Ability to work independently and as part of an interdisciplinary team
Job Summary:
Coordinates and completes clinical admissions for new residents in a timely and accurate manner.
Essential Job Duties:
- Perform comprehensive nursing assessments upon admission
- Review hospital records, discharge summaries, medication lists, and physician orders
- Initiate and document care plans based on resident needs
- Communicate effectively with physicians, interdisciplinary team members, residents, and families
- Ensure all admission documentation meets regulatory and facility standards
- Perform wound assessments, treatments, and dressing changes per physician orders
- Ensure compliance with infection control, safety, and quality assurance standards
- Maintain accurate and timely documentation in the electronic medical record (EMR)
- Participate in care plan meetings and interdisciplinary rounds as required
- Assist with audits, surveys, and regulatory compliance activities
- Support quality improvement initiatives related to admissions and wound care
