Position Summary

As a Start of Care Registered Nurse, you’ll provide skilled nursing care to patients in their homes, following physician orders and individualized plans of care. You’ll work independently while being fully supported by an experienced clinical leadership team.

This role is ideal for RNs who value autonomy, flexibility, and meaningful one-on-one patient relationships.

Requirements

Key Responsibilities

  • Complete Start of Care assessments and ongoing patient evaluations
  • Deliver skilled nursing services according to the plan of care
  • Educate and support patients and families
  • Coordinate care with physicians and interdisciplinary team members
  • Complete required documentation within agency guidelines
  • Supervise and support other nursing staff as needed
  • Participate in care conferences, peer review, and quality improvement initiatives

(On-call rotation and OASIS responsibilities apply per agency policy.)


Qualifications

  • Graduate of an accredited nursing program
  • Active RN license in the state(s) of practice
  • Home health or nursing experience preferred
  • Ability to work independently and exercise sound clinical judgment
  • Commitment to patient-centered care
