Title: Nurse Practitioner – Street Medicine
Department: Street Medicine Team
Reports To: Clinical Director, Street Medicine / Chief of Medicine
Summary
The Nurse Practitioner provides medical care to individuals served by the Street Medicine Team. The care environment ranges from working out of a backpack in the woods and in the street to working in our street medicine clinics. Responsibilities include primary and urgent care, encompassing assessment, diagnosis, treatment, and education. The provider practices with independence, sound clinical judgment, and adherence to evidence-based standards under the supervision of the Clinical Director and Chief of Medicine.

Qualifications
Education: Degree (Nurse Practitioner or Physician Associate) from an accredited Nursing or Physician Associate school. CPR Certification required.
Experience: Board Certification or Board Eligibility. (Board certification must be attained within the timeframe dictated by one’s specialty in order to remain on staff at the CS-HHC if one was originally Board Eligible at the time of one’s hire.) Connecticut Licensure. Must have at least 5 years of clinical experience in outpatient internal medicine or family practice. Prior experience working in addiction medicine preferred.
