POSITION SUMMARY
Assume primary health care of specific patients under the direction of the Medical Director. Follow established standards, procedures and practices, and gives specific patient care directions to nursing and other staff.
Colusa Medical Center provides both inpatient and outpatient services, including emergency services, to the surrounding community bringing exceptional healthcare and service to our community. We are looking for a Physician Assistant or Family Nurse Practitioners (NP, PA) to serve the primary care needs of our local rural community residents in Colusa, Williams, Arbuckle and nearby areas, California.

DUTIES AND RESPONSIBILITIES
In collaboration with supervising physician, duties may include, but are not limited to the following:
- Provide in-person patient care, including assessments, diagnosis, treatment planning, and follow-up.
- Conduct visits (triage, follow-ups, chronic condition support, patient education).
- Support patient access by managing urgent issues and communicating care plans clearly. Provide primary clinical care of clients of all ages in accordance with standardized procedures. Clinical care includes the following:
- Developing a problem list, a diagnostic, therapeutic and teaching plan, which includes preventive health.
- Patient education and counseling.
- Furnish and adjust medications in the formulary in accordance with the law.
- Order diagnostic studies such as laboratory and radiologic tests, as needed.
- Collaborate closely with physicians, MAs, and administrative staff to ensure seamless patient care.
- Maintain accurate, timely documentation in the EHR. Fills out all necessary forms.
- Participate in team huddles, quality initiatives, and workflow improvement efforts.
- Refer patients for specialty evaluation and care as needed.
- Review patient’s records, including laboratory data, for abnormalities and completeness and is responsible for providing appropriate follow-up.
- Oversee provider's inbox items including medication refill requests, lab/test results, and care-coordination tasks.
- Respond to or route items appropriately to ensure timely, accurate resolution.
- Escalate clinical concerns to supervising physicians when needed.

EDUCATION/TRAINING/EXPERIENCE:
Minimum requirements:
- Current BLS required. ACLS certification preferred.
- Current NP or PA license in California (Required)
- Current DEA license
