About the role

Sylva Valley Health & Rehab is hiring Licensed Practical Nurses (LPNs) to help strengthen our in-house team and provide high-quality care in a skilled nursing setting. We are looking for dependable nurses who value teamwork, strong resident care, and a stable work environment.

Key responsibilities

• Provide direct nursing care to residents according to physician orders and care plans
   
• Administer medications and treatments safely and accurately
   
• Observe and report changes in condition to the RN/provider as appropriate
   
• Document care clearly and timely
   
• Work closely with CNAs, nurses, providers, and the interdisciplinary team
   
• Help maintain a safe, respectful, resident-centered environment
   

Requirements

• Active Licensed Practical Nurse (LPN) license
   
• Skilled nursing / long-term care experience preferred
   
• Strong clinical skills, teamwork, and communication
   
• Dependable attendance and professional attitude
