What You’ll Do

  • Deliver compassionate, hands-on care navigation and skilled nursing for clients in their homes
     
  • Help older adults stay safe, healthy, and comfortable at home — avoiding unnecessary hospitalizations
     
  • Manage medications, coordinate care, and communicate closely with families
     
  • Perform skilled nursing services such as IVs, wound care, blood draws, and catheter care
     
  • Partner with leadership and medical teams to ensure exceptional client outcomes
     
  • Go beyond checklists — truly know your patients and make an impact every day

Requirements

What You’ll Bring

Active RN license (Required)

 

BLS and First Aid Certification

 

A caring heart, professional attitude, and passion for patient-centered care

 

Excellent communication and problem-solving skills

 

Experience with older adults or care coordination a plus

 

Other Requirements

Valid driver’s license & auto insurance

 

Current CPR, annual TB test, and flu vaccine

 

Clear background & drug screening

 

Salary Description
$60 hourly
