What You’ll Do

In this role, you will:

• Provide primary care for adult and geriatric patients with acute and chronic conditions 
• Perform comprehensive assessments, establish diagnoses, and develop individualized treatment plans 
• Prescribe medications and order/interpret diagnostic tests within scope of practice 
• Coordinate care with supervising physicians and interdisciplinary facility teams 
• Maintain accurate, timely, and complete clinical documentation 
• Collaborate with specialty providers (e.g., wound care, psychiatry) for integrated care delivery 
• Communicate clearly with patients, families, and care teams regarding care plans and changes in condition 
• Build strong, trust-based relationships with patients, families, and facility staff 
• Support consistent, high-quality care across both service areas 
• Perform other duties as assigned 

What You Bring

• Active Nurse Practitioner or Physician Assistant license in good standing 
• Active DEA registration 
• Current BLS certification 
• Strong clinical judgment and diagnostic skills 
• Ability to work independently in structured care settings 
• Strong communication and interdisciplinary collaboration skills 
• Commitment to compassionate, patient-centered care
