What You’ll Do
In this role, you will:
- Provide primary care for adult and geriatric patients with acute and chronic conditions
- Perform comprehensive assessments, establish diagnoses, and develop individualized treatment plans
- Prescribe medications and order/interpret diagnostic tests within scope of practice
- Coordinate care with supervising physicians and interdisciplinary facility teams
- Maintain accurate, timely, and complete clinical documentation
- Collaborate with specialty providers (e.g., wound care, psychiatry) for integrated care delivery
- Communicate clearly with patients, families, and care teams regarding care plans and changes in condition
- Build strong, trust-based relationships with patients, families, and facility staff
- Support consistent, high-quality care across both service areas
- Perform other duties as assigned
What You Bring
- Active Nurse Practitioner or Physician Assistant license in good standing
- Active DEA registration
- Current BLS certification
- Strong clinical judgment and diagnostic skills
- Ability to work independently in structured care settings
- Strong communication and interdisciplinary collaboration skills
- Commitment to compassionate, patient-centered care
