Nurse Liaison - Non-Medical Home Care Services (Full-time)

Location: Fort Wayne, Indiana (serving clients throughout Northeast Indiana)

Our client is a regional provider of non-clinical, in‑home care services.  Their primary client base includes older adults seeking to maintain independence while receiving support with daily living activities, companionship, and cognitive or memory-related care. Services are particularly relevant for individuals living with dementia, those transitioning home from hospital or rehabilitation settings, and families looking for supplemental caregiving support.  We are seeking a compassionate and relationship-focused Nurse Liaison to serve as a resource for clients, families, referral partners, and internal care teams.

Position Overview

The Nurse Liaison plays an integral role in evaluating client needs, developing customized care recommendations, and supporting families through important care decisions. This position combines clinical expertise, relationship management, and ongoing care oversight to help ensure clients receive exceptional support as their circumstances evolve.

Primary Responsibilities

Client Evaluations & Care Planning

  • Meet with prospective clients and families to discuss care needs, goals, and concerns.
  • Complete in-home assessments to evaluate health status, safety considerations, functional abilities, and environmental factors.
  • Design individualized care plans that reflect each client's preferences, routines, and support requirements.

Ongoing Clinical Oversight

  • Review client progress and make recommendations when care needs change.
  • Partner with care coordination staff to ensure services are delivered effectively and consistently.
  • Provide clinical insight and support regarding client care concerns and service delivery.

Family & Client Support

  • Guide families through available care options and service expectations.
  • Act as a trusted resource during care transitions and changing health circumstances.
  • Connect clients and families with appropriate community and healthcare resources when needed.

Community & Healthcare Relationships

  • Develop strong professional relationships with physicians, hospitals, rehabilitation providers, hospices, senior living communities, and other referral sources.
  • Collaborate with healthcare professionals to promote continuity of care and positive client outcomes.
  • Represent the organization within the Northeast Indiana healthcare community.

Service Quality & Client Satisfaction

  • Conduct periodic follow-up visits and reassessments to ensure care remains aligned with client needs.
  • Evaluate client and family satisfaction and address concerns promptly.
  • Support initiatives that enhance service quality, client experience, and caregiver performance.

Qualifications

  • Active Indiana RN or LPN license.
  • Experience working with older adults in home care, hospice, senior services, case management, discharge planning, or related healthcare settings preferred.
  • Strong clinical assessment and critical thinking abilities.
  • Exceptional communication and relationship-building skills.
  • Ability to work independently while managing multiple priorities.
  • Proficiency with documentation and care planning processes.
  • Valid driver's license and willingness to travel throughout Northeast Indiana.
